Advanced Nurse Practitioner Ft Or Pt Band 8a

Overview

Horton Park Medical Practice is seeking a motivated and experienced Advanced Nurse Practitioner to join HORTON PARK MEDICAL PRACTICE's friendly clinical team. This role involves providing acute care for patients through face-to-face, video, and telephone consultations, as well as home visits when necessary. You will be responsible for the complete patient care process, from initial history taking and clinical assessment to diagnosis, treatment, and evaluation of care.

Main Duties

The successful candidate will demonstrate clinical decision-making and expert care, working collaboratively within the practice team to meet patient needs. Responsibilities include:

• Providing patient care within professional boundaries
• Managing daily demand in collaboration with GPs
• Supporting policy and procedure delivery
• Developing leadership within a clinical practice area

Person Specification

Experience

• Experience in a community setting
• Experience managing minor ailments
• Desirable: experience in a GP practice, nursing across chronic diseases, palliative care experience

Qualifications

• Essential: Recognised Advanced Nurse Qualification (MSc), post-registration experience
• Desirable: Primary Care Experience, nurse-led management of minor illness, diabetes and respiratory diplomas
