Live Connected is an exciting branch of the Connected Health group. Live Connected provide quality care to adults with mental ill health, learning disabilities, dementia, and complex needs, through supported living.

KEY DUTIES AND RESPONSIBILITIES

  • To undertake daily living tasks as agreed with the service user, their family, and the professionals involved with the individual service plan.
  • To always work within the policies and procedures of Connected Living.
  • To always work alongside the care and risk assessments within the service specific to each of the service user's needs.
  • To complete Health and Safety checks within the service in line with the policy and procedures in place.
  • To be confident to lone work in line with the lone worker risk assessment.
  • To ensure safe medication is delivered and ordered to the service users' homes.
  • To provide service users with opportunities to express their preference as to the way tasks are carried out.
  • To maintain and promote choice and independence to the service users Live Connected work with.
  • To manage service users' behaviors in the least restrictive way possible.
  • To ensure all daily notes, handovers, and any relevant documentation are completed by the end of the shift, and high-quality efficient handovers are provided to the oncoming shift.
  • To work as part of a team and wider multidisciplinary team to support the clients' emotional and physical wellbeing.
  • To develop and maintain professional working relationships with service users and the relevant multi-disciplinary team to ensure positive outcomes for the service user.

Criteria

  • 6 months experience within Supported Living Service
