Overview

Kent and Medway NHS and Social Care Partnership Trust is looking for a CRHT Support Worker to join Kent and Medway NHS and Social Care Partnership Trust's established Crisis Resolution and Home Treatment Team. You will support patients who are acutely mentally unwell in the community and provide the best level of care while they remain in their own homes. Kent and Medway NHS and Social Care Partnership Trust's services include:

  • Support for individuals experiencing acute mental health episodes
  • Assistance for patients discharged from inpatient wards

You will work within the Rapid Response function, undertaking assessments 24/7, alongside a qualified team member from the Urgent Mental Health Helpline, including gatekeeping assessments. This role offers you the chance to be part of a dynamic service that fosters skill development and career advancement.

Responsibilities include:

  • Working collaboratively within a multi-disciplinary team
  • Delivering interventions aimed at promoting recovery such as:
    • Health promotion
    • Anxiety management
    • Social inclusion
    • Emotional first aid
    • Self-harm reduction
    • Safety planning

This is a permanent role under the Agenda for Change band. The service operates on a 24-hour shift rotation across 7 days a week.
