Overview

Hospital at Home Frailty ward has an exciting opportunity for a practitioner with advanced skills to join the team. Somerset NHS Foundation Trust have 2 full-time posts available, one in Mendip area and another in South Somerset.

As a compassionate registered professional with advanced clinical skills, you will work within a hospital-at-home team, understanding the services offered to patients in Somerset to help them stay in their own home environment.

The service covers urban and rural areas of Somerset. While predominantly based in one area, cross-cover, especially on weekends, will be required. Hospital at Home operates between 8 am and 8 pm, providing a variety of shifts within Somerset NHS Foundation Trust's rota. Flexible working solutions are offered to ensure a work-life balance, with weekend cover as part of the role. The minimum contract hours for this role are 30.

Main Duties of the Job

  • Enable patients to receive acute care and treatments in their home surroundings with support from health and care professionals.
  • Attend MDT and huddles to discuss patient caseload and plan interventions.
  • Carry out autonomous assessments and treatment plans, managing patient expectations.
  • Maintain open and honest communication with patients, families, and stakeholders.
  • Prioritize time and support the team to manage caseloads effectively.
  • Build relationships with neighborhood teams to support patients in their homes.
  • Work collaboratively with internal and external teams to promote the hospital-at-home initiative.
  • Participate in service development and identify personal and team training needs.
  • Ensure compliance with the four pillars of the ACP role.
