Overview

Job Context

Working in a local community social care team, the main focus of the role will be supporting the Team Manager with quality assurance, team performance, coordination of safeguarding, and complex casework. You will work as part of the team under the Service Manager in a specific locality or team. The focus of the Advanced Practitioner in the Service Manager locality is to implement the requirements of the Care Act with a focus on quality, performance, practice, and outcomes for people.

Job Purpose

To build upon the successful implementation of personalisation and self-directed support in Adult Social Care and support locality teams in improving their performance and outcomes for people. The role is integral to improving and monitoring the quality of social care/social work practice and the implementation of strength-based approaches to assessment, review, and support planning.

Responsibilities:

  • Work with specific teams ensuring that practice is compliant with relevant legislation, is safe, and of good quality.
  • Develop staff awareness and practice regarding performance and quality.
  • Ensure the compliance of designated operational teams with various performance requirements.
  • Undertake quality monitoring of adult social care/social work practice through the review of assessment and support planning.
  • Report any areas of concern around practice or performance to the Team Manager and Service Manager.
  • Lead on a program of audit and review of activities across the Directorate to identify improvements in performance.
  • Provide day-to-day information, advice, and guidance to staff within a specific locality.

Operational Management:

Ensure locality and performance teams have processes in place to support the Performance framework and highlight interventions and trends in social care/social work practice. Ensure practice meets desired outcomes for people and that their views and opinions are reflected in social work practice.

Resource Management:

  • Identify and report any areas of concern in relation to practice, ASYE arrangements, or performance.
  • Ensure that all staff and managers understand their roles regarding the performance and quality agenda.

Partnerships:

Establish and develop positive and supportive relationships with relevant staff at all levels within the council, health service, and other partners to promote effective joint working arrangements.

Communications:

Communicate effectively with other practitioners and professionals to ensure required outcomes are achieved. Provide professional information, advice, and guidance as required.

Systems and Information:

Ensure that practitioners utilize current business processes to support Adult Social Care in relation to case recording, quality, performance, and financial monitoring.

Safeguarding:

Be committed to safeguarding and promoting the welfare of children, young people, and adults, raising concerns as appropriate.
