Care coordinators play a crucial role within the Primary Care Network (PCN) by proactively identifying and assisting individuals, including the elderly and those with long-term conditions, in navigating their care and support across health services.
Main Responsibilities
The successful candidate will be situated in Woodberry Practice's PCN Hub at The Woodberry Practice. They will be dedicated, reliable, and person-focused, thriving on interaction with a diverse population. Key responsibilities include:
- Supporting PCN practices with Health Action Planning.
- Managing appointments and ensuring access to quality information for care choices.
- Collaborating with GPs and primary care professionals to manage a caseload, including patients with long-term health conditions.
- Assisting with immunisations and clinic bookings.
- Working with the PCN manager on various duties and quality delivery initiatives.

Working Pattern
This is a full-time position with the flexibility of home or remote working, accommodating a healthy work-life balance.
Qualifications
Essential:
- Educated to GCSE or Equivalent (A-C).
- Experience using the clinical system - EMIS.
- An interest in primary care.
Experience

- Experience working in general practice.
