Overview

Southern Health NHS Foundation Trust is looking for an enthusiastic Community Mental Health Nurse to join Southern Health NHS Foundation Trust's friendly multi-disciplinary team as a Care Co-ordinator. Job Summary

You will hold a caseload and undertake the role of care coordinator for those with varied complex mental health and social care needs. You will provide assessment and specific interventions to meet the needs of service users, their carers, and families. It is expected that you will be a flexible practitioner who enjoys new challenges in meeting the developing needs of the service and Southern Health NHS Foundation Trust's service users.

Main Duties, Tasks & Skills Required

  • Hold a caseload as a care coordinator.
  • Provide assessment and specific interventions.
  • Work proactively with multi-agencies.
  • Flexible working hours with a schedule of Monday to Friday, 9 am - 5 pm, with occasional weekends or bank holidays.

Qualifications

Essential

  • Registered Nurse, Social Worker, Mental Health Practitioner or Occupational Therapist.
  • Significant post-qualifying experience.
  • Mentorship course completion.
  • Clinical supervision training.
