Care Coordinator

The successful candidate will play a key role in proactively identifying and working with people who engage with the practice irregularly and those who fail to engage with smear programmes and child vaccinations. They will develop relationships with hard-to-reach groups to better meet their needs.
Key responsibilities include:
- Identifying and assisting individuals who regularly miss appointments, particularly the frail and elderly.
- Managing queries, appointments, and providing quality information to aid decision-making.
- Working closely with GPs and practice teams for appropriate clinical support.
- Coordinating care across health and social services, ensuring a joined-up approach.
- Signposting to external organizations for additional support.
- Attending multidisciplinary meetings for effective patient follow-up.
- Supporting unpaid carers in accessing relevant services.
The ideal candidate will have excellent interpersonal and communication skills, be organized, patient, and empathetic, with experience in health or social care roles. This non-clinical role requires someone who is dedicated, reliable, and enjoys working with diverse populations.
