The Care Home Support Team (CHST) has an exciting opportunity for you to be part of their dynamic team which visits and supports local care homes across the Borough of Richmond.

You will work with residents and care home staff to assess residents' needs, putting a plan of care in place, and delivering training with the purpose of reducing avoidable ambulance call outs and hospital admissions.

Hounslow and Richmond Community Healthcare NHS Trust is looking for a self-motivated, autonomous, proactive registered nurse committed to their professional development, bringing their skills and knowledge to the team.

You will be based in the community, working in collaboration with a network of health and social care professionals.

Responsibilities

• Ensure timely provision of high standards of assessment, advice, planning, implementation, and evaluation of care.
• Work as an autonomous practitioner, being professionally and legally accountable for all aspects of professional and clinical work.
• Collaborate with and support members of the MDT in managing complex health and social care issues within care homes.
• Contribute to the development of CHST, ensuring the provision of an equitable, responsive, high quality service.
