Search Consultancy LTD is actively seeking skilled District Nurses to join Search Consultancy LTD's team, supporting NHS patients within their own homes and the community.

Duties:

• Community Nursing Care: Providing essential nursing care to patients in their own homes, including wound care, administering medications, and managing chronic conditions.
• Patient Assessments: Conduct assessments of patients' health needs, develop care plans, and monitor their progress.
• Health Education: Educate patients and their families on managing health conditions, medication, and lifestyle changes to improve health outcomes.
• Collaborative Care: Work closely with GPs, other healthcare professionals, and social care teams to deliver integrated care.
• Home Visits: Conduct home visits to assess patients' living conditions, provide support, and identify any potential health or safety risks.

Requirements for the Role:

• NMC Registration
• Experience in District/Community nursing is essential for this post, as well as excellent community skills.
• Occupational Health and DBS clearance will be requested prior to working.
