Health and Social Care Coordinator - Part-time

Overview

Kent Community Health NHS Foundation Trust is looking for a skilled and compassionate Health and Social Care Coordinator to join Kent Community Health NHS Foundation Trust's team based in Maidstone. Working as part of Kent Community Health NHS Foundation Trust's Adult Services team, Kent Community Health NHS Foundation Trust's Health and Social Care Coordinators support patients within each defined primary care network population with social prescribing.

The patients will routinely be the top 3% Long Term Condition patients with complex needs. As a Band 4 Health and Social Care Coordinator you will:

• Support housebound patients living with a Long Term Condition to effectively manage and improve their well-being.
• Reduce unnecessary hospital attendances and subsequent admissions.
• Support hospital discharges from Acute and specifically Frailty Units.
• Respond to referrals within an appropriate time frame and refer to other services as appropriate.
• Support inter-service referrals.
• Provide advice and support to patients, carers, and staff.
• Manage a caseload of patients, plan expectations, and recognize outcomes.
• Create Personalized Plans for each patient and support the MDT with an advocacy role.
• Ensure good communication to lead.

Additional Working Pattern: 8-4, 4 days/week.
