The role of a community Healthcare Support Worker is to visit patients at home to provide nursing interventions under the direction of registered nursing colleagues. The focus of the role is to support admission avoidance, promote independence, and assist in the effective management of patients with long-term conditions.
Responsibilities
- Work within the multi-disciplinary Integrated Care Team delivering community-based services to a GP Practice population.
- Undertake nursing tasks within agreed parameters set out by registered practitioners.
- Conduct home visits and work independently without direct supervision.
- Support admission avoidance, early supported discharge, and promote independence.
- Assist in managing patients with long-term conditions and older people with mental health problems.
Qualifications
Essential
- Literate and numerate.
- Relevant experience as a care assistant in a healthcare environment.
- Evidence of delivering care delegated from a range of registered professionals, particularly nursing.
- Ability to implement prescribed programmes of care for various conditions.
- NVQ 3 in a healthcare-related subject or willingness to undertake.
Working Conditions
Hours: 37.5 hours per week.
