Overview

Suffolk County Council have an opportunity for a registered Health or Social Care professional to join Suffolk County Council's Ipswich West Integrated Neighbourhood Team as an Integrated Neighbourhood Team Manager (INT Manager).

Based in the stunning county of Suffolk, you will hold a key role in supporting the development of integrated working across Health and Social Care through the development and management of the Integrated Neighbourhood Teams, including the associated Connect. You will have joint accountability to Adult Social Care in Ipswich and East Suffolk and to East Suffolk and North East Essex Foundation Trust (ESNEFT).

Collocated in Constantine House Ipswich, this role covers the geography of West Ipswich.

The main purpose of your role will be to develop, directly manage and lead the Integrated Neighbourhood Team, reporting to Senior Management teams across both organisations. Your responsibilities will include:

  • Leading specific clinical/care governance activity, including the setting and monitoring of standards and performance.
  • Responding to complaints, staff support, learning, development, supervision, and the implementation of policy and clinical guidelines.
  • Overseeing the implementation of the health and care strategy at the neighbourhood level.
  • Working closely with all partners, including primary care, acute care, specialist services, mental health services, housing, the voluntary sector, and care providers.

The Team:

There are 8 Integrated Neighbourhood Teams across Ipswich and East Suffolk, bringing Health and Social Care Services together to provide a single coordinated care response underpinned by prevention, self-care, early intervention, reablement, and rehabilitation rather than longer-term treatment and lifelong service dependency.

This role will be key in building relationships within the Connect community.
