The team aims to proactively provide assessment, potential diagnosis, and signposting to other services for individuals aged 65 and over referred to the service with cognitive impairment. Hampshire and Isle of Wight Healthcare NHS Foundation Trust work closely with care facilities, individuals, and families in their homes, alongside GP liaison and Hampshire and Isle of Wight Healthcare NHS Foundation Trust's NHS Dementia Service.
Main Duties
- Ensure comprehensive assessments of needs and risks.
- Plan and deliver care for individuals requiring acute responses.
- Collaborate with service users and other agencies.
- Promote holistic, person-centered care.

Job Responsibilities
This role involves risk assessments and planning intensive support for individuals in need. The post holder will carry registration as either a Registered Nursing Associate or possess equivalent qualifications. Effective communication, sensitive handling of confidential information, and interdisciplinary collaboration are crucial.
Person Specification
Qualifications
- Registered Nursing Associate or Care Certificate Level 3.
- Knowledge of relevant legislation in health care.
Experience
- Experience of reflective practice and ongoing risk assessment.
- Therapeutic engagement with patients.
- Knowledge of various models in social and health care.

- Excellent verbal and written communication skills.
