JOB DESCRIPTION

Purpose of the Post: The Nurse Associate post aims to provide exceptional nursing care and support to clients accessing Middlesbrough Council's service, many of whom are vulnerable and/or have complex needs. This role involves working collaboratively to assess patient needs and implement clinical interventions.

Duties and Responsibilities:

• Provide compassionate and non-judgmental nursing care to individuals experiencing multiple needs.
• Collaborate with the team to assess client needs and develop individualized care plans.
• Undertake clinical tasks while adhering to service protocols and professional standards.
• Monitor client progress and adjust care plans as necessary.
• Inform clients of treatment options and provide harm reduction strategies.
• Identify and assess safeguarding concerns and raise them appropriately.
• Coordinate referrals to healthcare providers and community resources.
• Commit to continuous professional development and partake in quality improvement initiatives.
• Maintain accurate and timely documentation of client interactions.
• Understand and act in line with NMC professional standards.
