About the Role

Church Street Partnership has a fantastic opportunity for an experienced practice nurse, or qualified registered nurse to join our team on a part-time basis, for 25 hours per week. Hours and days of work able to be negotiated. 

The main responsibilities of the role include:

  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Diagnosing and treating patients presenting with minor illness and treating them following practice guidelines and protocols, with support from practice GP's
  • Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Managing and treating long-term conditions, in line with national and local policies and the practice needs.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Supporting and managing the health needs of women presenting for family planning and cervical cytology consultations.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Hypertension review and management.
  • Running asthma, COPD and diabetes clinics and reviews.
  • New patient and health checks.

About the Candidate
All applicants should have the following qualifications, registrations, skills and experience:
  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications; wewould be willing to consider a candidate with suitable experience for the role, whilst they do the FoPN course alongside.
  • Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.  The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.

The role is based in Bishop's Stortford, Hertfordshire, which has been rated as one of the best places to live within the UK. Bishop's Stortford is a vibrant market town with close links to Stansted Airport, London and Cambridge the M11 and M25. Church Street Partnership is a large 3 branch practice with 6 GP Partners and almost 16,500 registered patients. The practice is rated 'Good' by the CQC. 

The practice is an active member of Stort Valley and Villages Primary Care Network, bringing together 5 local GP surgeries across Stort Valley & Villages, serving around 66,000 patients. With the combined workforce, skills and knowledge, we are in a unique position to assist our patients across the county.
