Peterborough City Council operates a reablement service dedicated to helping individuals regain independence in their daily activities. Peterborough City Council's goal-based assessments are designed to support clients in achieving their independence, utilizing technology-enabled care devices and occupational therapy aids.
Peterborough City Council prioritise client interaction while also providing supervision and mentoring to Peterborough City Council's dedicated staff.
Key Responsibilities:
- Conduct goal-based assessments for new clients entering Peterborough City Council's Reablement Service.
- Work closely with health and social care colleagues to support client discharges from acute hospitals.
- Provide supervision and mentoring to Reablement support workers delivering care to vulnerable clients.
- Support clients in utilizing technology-enabled care devices and occupational therapy aids.
- Maintain knowledge of care practices, triage skills, risk assessment, CQC standards, and the Care Act.
This role includes a 9-week rota with a total of 3 weekends.
