Summary
Midlands and Lancashire CSU is looking for qualified Senior Practitioners to join the Personalised Healthcare Commissioning (PHC) service to support Integrated Care Boards (ICBs) in providing care for patients receiving health funding. The post holder will play a key role in delivering an effective and competent level of service and consistently provide a client-focused experience.
Responsibilities
- Work within a team of Personalised Healthcare Assessors, leading complex cases and ensuring high-quality service delivery.
- Manage a caseload of patients with complex health needs.
- Facilitate multi-disciplinary assessments and patient reviews according to standard operating procedures.
- Support the PHC team to fulfill workstream plans effectively.
- Mentor and supervise PHC Associate Practitioners, Practitioners, and students.
- Communicate risks, issues, and dependencies through briefings and reports.
- Ensure compliance with NHS Continuing Healthcare National Framework.
Qualifications and Skills

- A full UK driving licence and access to a vehicle.
